For thirty years, creatine got marketed to one customer: the guy in the corner of the gym trying to add an inch to his arms.
Meanwhile, the group with the lowest creatine stores in their body, the group whose diets supply the least of it, and the group whose research is finally starting to show the widest range of benefits — women — got told it would make them bulky, bloated, and heavy.
That was never true. And the science has moved a long way past it.
What creatine actually is
Creatine isn't a hormone. It isn't a steroid. It isn't a stimulant.
It's a compound your body already makes — in your liver, kidneys, and pancreas — from three amino acids. You also get it from food, mostly red meat and fish. It's stored primarily in your muscle, with a meaningful amount in your brain.
Its job is energy. Specifically, it helps your cells rapidly regenerate ATP, the fuel your body burns for short, intense demands. Lifting something heavy. Standing up fast. Sprinting for a train. Thinking hard on four hours of sleep.
Creatine doesn't create energy. It helps you recycle it faster. That's the whole mechanism, and it applies to every cell that runs on ATP — which is all of them.
Why women start behind
Research suggests women carry roughly 70–80% lower total creatine stores than men. Part of that is body composition. Part of it is diet: women, on average, eat less red meat and fish, the two richest dietary sources. And anyone eating vegetarian or vegan is getting close to none.
So the starting line is different. Which means the room to gain from topping those stores up may be wider — not narrower.
The hormone connection — what's actually true
This is where a lot of supplement marketing gets sloppy, so let's be precise.
Creatine does not regulate your hormones. It doesn't balance estrogen, fix your cycle, or treat anything.
The relationship runs the other direction: your hormones influence your creatine.
Estrogen and progesterone affect the enzymes your body uses to synthesize creatine, and they influence creatine kinase — the enzyme that puts creatine to work. As those hormones shift across the menstrual cycle, through pregnancy, and through perimenopause and menopause, your body's creatine metabolism shifts with them.
That's the real argument. Not that creatine does something to your hormonal system, but that a hormonal system in flux places changing demands on your creatine stores — and those stores were already lower to begin with. Supplementing is about keeping the tank full while the demand moves around.
Honest bottom line: this is an active area of research, not settled science. The mechanism is well established. The downstream real-world effects are still being mapped.
Bone density and midlife
This is the part worth the most attention, and also the part most often oversold.
After menopause, bone loss accelerates. It's one of the defining health challenges of the second half of a woman's life.
The most cited work here is a two-year trial in postmenopausal women that combined creatine supplementation with resistance training. The group taking creatine alongside their training maintained bone in the hip and showed favorable changes in bone geometry compared to the group training without it.
Note the critical detail: it was creatine plus resistance training. Studies that gave creatine without a training stimulus have generally not found bone benefits.
Creatine is not a bone pill. It appears to help your body respond better to the thing that actually builds bone — loading it. The lifting is the medicine. Creatine may help you do more of it, recover better from it, and get more out of each session.
If you take one thing from this post: if you're a woman over 40 and not doing resistance training, that's the first move. Creatine is the multiplier, not the mechanism.
The part nobody talks about: your brain
Your brain uses a large share of your body's energy, and it runs on the same ATP system your muscles do.
Creatine research in the brain is younger than the muscle research, but it's building fast. Areas showing promise include cognitive performance under sleep deprivation and stress, mood — including work on creatine alongside standard treatment in women with depression — and recovery from cognitive fatigue.
Anyone who's parented a toddler through a bad night, worked a double, or gone through a hard season knows what running on empty feels like. The idea that a compound supporting cellular energy might matter there isn't a stretch. It's just an area where the research is still catching up to the mechanism.
The myths, quickly
"It'll make me bulky." Creatine doesn't build muscle on its own. It helps you train harder, and training builds muscle — at a rate governed by your hormones, your calories, and your effort. Women don't accidentally get big.
"It causes bloating." The water creatine draws goes primarily inside muscle cells, not under your skin. The bloating reputation comes from old-school loading protocols using 20g+ a day. Skip the loading phase and the issue largely disappears.
"It's hard on your kidneys." In healthy people, decades of research haven't supported this. If you have existing kidney disease, that's a conversation with your doctor — as it would be with any supplement.
"It causes hair loss." This traces back to a single small study in rugby players that measured a hormone marker, not actual hair loss, and has not been replicated since. It's one of the most repeated claims in fitness with some of the thinnest evidence behind it.
How to actually take it
Creatine monohydrate. Not HCl, not buffered, not a blend with eleven other things and a proprietary label. Monohydrate is the form nearly every study used, and it's the cheapest. Look for Creapure or a third-party tested product.
3–5g per day. Every day, including rest days. It works by saturating your stores over time, not by hitting you in the moment.
No loading phase needed. Loading saturates you in about a week instead of three to four. It also causes most of the stomach complaints. Unless you're in a rush, skip it.
Timing doesn't matter much. Consistency does. Put it in your coffee, your water, your smoothie. Whatever you'll actually do daily.
A note on brain-focused dosing: some of the cognitive research uses higher amounts than 5g. That's still being worked out. Start at the standard dose.
